I think it is again result of not respecting the original design: The resistors are required (by their datasheet) to have >5mm spacing to any conductive object or they could be sleeved by high heat resistant sleeve (= in real life the glass reinforced) And their datasheet mention than incandescent particles may fly from the resistor body during the fusing event, so the design should be made resistant, again the glass reinforced sleeve does the job... So here (what you describe) someone saved a cent on the sleeve... Well, I have seen this mainly on "repaired" stuff, together with non-fusible resistor or use of simple glass fuses instead of the sand filled ceramic ones. However the standard resistor (or wrongly made attempt for the fusible type) used in place of the really certified fusible one is quite common problem with new devices, not only repairs...
