I was looking for Philips Alto F20T12 K&B lamps and happened to come across this...

I think the fixture is a Dazor but I'm not sure. I thought it was the 135C3 model but then I realized that the lamps were 24 inches long. Either way, an odd combination converting a preheat desklamp to LED, and using "home" photos for the listing!
