The date looks like January 1941.
This information is the best I have ever seen regarding early fluorescent lighting.
There is a picture and description of the first type of starters that were used before the twist in starters.
You may have to refresh the screen to get it to work.

I'll add that for anyone who isn't aware, here's a great source of vintage lamp literature, including three GE lamp catalogs from 1937, 1948, and 1960. Would you believe the first F4T5 lamps were rated for only 500 hours!
